So as a dm I've learned the scariest thing you can do to your players is just roll behind the screen with no explanation. So after one of my players got a new fancy sword, anytime there was a lull in the game I'd roll. I set my dc at 10, and was rolling hot for a while. Three sessions later, I finally failed, looked at the fighter and said "your sword coughs"
